Southern Oregon Coast Dental Health Survey
Please help us by answering these questions about your dental health. You are being asked to answer the survey because you are an adult living in Curry County or Coos County. We want to know about your experience getting dental care, what prevents you and what helps you get dental care, and your dental health. This information will help the South Coast Oral Health Coalition plan for services to improve dental health in the counties.

What does dental health mean? Dental health is a picture of the health of your mouth. This includes your teeth, gums, soft upper inside of your mouth, and your throat, tongue, chewing muscles and jaw.
